Lens-based glaucoma surgery: using cataract surgery to reduce intraocular pressure.

چکیده
UNLABELLED Cataract surgery has been demonstrated to lower intraocular pressure (IOP) in eyes with glaucoma. The evidence is strong in angle-closure patients that cataract surgery improves IOP and dramatically reduces the future risk for IOP spikes and acute attacks. Randomized controlled trials (RCTs) have shown that cataract surgery may be preferable to laser iridotomy or phacotrabeculectomy. The data are so convincing that an RCT has been undertaken to assess whether clear lens extraction may be appropriate following acute attacks and in cases of uncontrolled IOP. The case favoring cataract surgery in open-angle glaucoma (OAG) is more controversial. Impressive IOP reduction has been shown in OAG patients when the patients are stratified by preoperative IOP. The patients with the highest preoperative IOPs have pressure reductions up to 8.5 mm Hg. However, these findings have been criticized for methodological weaknesses. Combining cataract surgery with microinvasive glaucoma surgery in phaco-plus procedures provides further options for lens-based glaucoma surgery. Intraocular Lens Implantation in Phacomorphic Glaucoma
Method: Cataract surgery was performed after controlling the intraocular pressure in patients presenting with acute phacomorphic glaucoma. Intraocular lenses were inserted in the bag after conventional extracapsular cataract extraction. Patients with minimum follow up of one year were analyzed for the study. Corrected visual acuity, intraocular pressure and any complications of surgery were rec...
متن کامل1 Prognosis of Filtering Operations after Phacoemulsification
Intraocular surgical procedures in general and filtering surgery in particular, predispose to cataract formation or accelerate pre-existing lens opacification thus the management of a visually significant cataract in a glaucoma patient who has had a previous trabeculectomy is a common clinical challenge, with the possibility that the intraocular surgery might result in filtration failure and lo...
متن کاملComparison of intraocular pressure fluctuations before and after ab interno trabeculectomy in pseudoexfoliation glaucoma patients
PURPOSE Ab interno trabeculectomy (AIT) with the Trabectome has been shown to reduce intraocular pressure (IOP) in eyes with pseudoexfoliation (PEX) glaucoma. Here, we examined the change of IOP fluctuations before and after only AIT or AIT with cataract surgery in PEX patients using the contact lens sensor Triggerfish®. METHODS This was a prospective open-label study. Twenty-four consecutive...
متن کاملGoniosynechialysis and Repositioning of Intraocular Lens in a Case of Secondary Angle Closure Glaucoma.
PURPOSE Implanting intraocular lens (IOL) in proper direction is important in cataract surgery to reduce the possibility of undesirable complications. We experienced a case who underwent vitrectomy combined with cataract surgery and developed secondary angle closure glaucoma caused by IOL misdirection. Goniosynechialysis (GSL) and repositioning of IOL successfully ameliorated the high intraocul...
متن کاملTreatment of ophidism-induced bilateral canine cataract by extracapsular lens extraction and intraocular lens implantation
The aim of this study was to report a new cause of cataract, namely snake bite induced cataract in a dog. A 3-year-old male mongrel dog was referred to our clinic for the assessment of cataract in both eyes. The dog was bitten by a snake nearly 4 months ago. A 40 dioptre acrylic lens with 13 mm haptics was implanted into the lens capsule. In the left eye, posterior synechia was detected in the ...
